I/we declare that to the best of my/our knowledge and belief, the information I/we have given is correct and complete in every detail and I/we have not withheld any material fact. I/We understand that in assessing this application, CRL Management Ltd, its Principal or its Underwriter may search the files of Credit Reference Agencies who will record any credit searches on my file. I/We hereby consent to CRL Management Limited carrying out these searches, as it deems necessary. I/we understand that the signing of this form does not bind us to effecting insurance with CRL Management Limited but agree that should a quotation for cover be accepted, that this proposal and the statements made therein shall form the basis of the contract between me/us and the Underwriter.

I/We undersigned certify that all details in this form are complete and true. To My/Our knowledge no material information has been voluntarily withheld or omitted. I have read and understand The Terms and Conditions of Membership (The Rules). I/We understand and confirm that Construction Management Limited are entitled to refuse Membership Application.

Acknowledgement and acceptance of our terms and conditions is made by you signing/submitting a membership form and/or accepted quotation. By signing/submitting this form and/or quotation you accept responsibility for paying the membership fee and/or premium quoted in the accepted quotation. The membership fee and/or premium due as per accepted quotation must be paid upon remittance of signed membership form and/or accepted quotation. Failure to do so will result in the company being entitled to use all remedies provided by law to seek settlement of the debt. By signing/sending this form, consent is also being given for an electronic/credit search to be undertaken by CRL Management Limited on the identities disclosed in the membership application, should it be deemed necessary in order to progress the membership application.
